About the Role
As the Team Leader – Aged Care, you will oversee the delivery of aged care services, ensuring compliance with Commonwealth Home Support Program (CHSP) guidelines and aged care standards. You will be responsible for client assessments, care planning, team leadership, rostering, remote service coordination, and quality assurance.

Position Responsibilities
- Lead and support a team of aged care staff, ensuring high-quality service delivery
- Oversee client assessments, care planning, and service reviews
- Manage team rosters for efficient workforce utilisation
- Ensure compliance with CHSP guidelines, My Aged Care referrals, and funding agreements
- Monitor service quality and implement continuous improvements
- Provide mentorship, training, and professional development to staff
- Ensure compliance with risk management, safety, and aged care regulations
What we are looking for?
Essential Criteria:
- Tertiary qualifications in Aged Care, Home Care, Disability, or a related field
- Experience working with Indigenous and culturally diverse communities
- Knowledge of NDIS frameworks and aged care funding models
- Demonstrated experience in aged care service coordination or leadership
- Proven ability to mentor and support a team of care workers
- Excellent communication and interpersonal skills
- Current NT Driver’s License, Ochre Card, Criminal History Check, NDIS Worker Screening Clearance, First Aid & CPR Certification.
Desirable Criteria
- Knowledge of CHSP guidelines, My Aged Care portal, and aged care compliance
- Experience in care planning, client assessments, and service delivery
- Proficiency in CRM systems, Microsoft Office, and aged care reporting
- Ability to manage remote service delivery and liaise with external providers
